Fibrosis Screening Index (FSI)

A simple first-line tool based on routinely available clinical variables to support liver fibrosis risk assessment in populations targeted for fibrosis case-finding.

Target population

The FSI is intended for adults with one or more of the following risk factors:

  • Type 2 diabetes mellitus
  • Obesity with additional cardiometabolic risk factors
  • Elevated liver enzymes
  • Imaging evidence of hepatic steatosis

FSI = −15.269 + 0.032 × Age + 0.544 × Diabetes status + 0.197 × BMI + 1.039 × ln(AST) + 0.585 × ln(GGT) + 136.396 × (1 / Platelet count)

Age is expressed in years, BMI in kg/m², AST and GGT in U/L, and platelet count in ×10⁹/L. Diabetes status is coded as 0 (no) or 1 (yes). ln denotes the natural logarithm.

Clinical interpretation

Clinicians can use the FSI to assess fibrosis risk and identify individuals who may benefit from further evaluation or specialist referral. Individuals classified as low risk may continue routine clinical management and follow-up. Individuals classified as intermediate or high risk may benefit from further assessment with VCTE and specialist evaluation when clinically appropriate.
